    I have been involved in providing home health care to seniors for the past 22 years and I have enjoyed every minute of it! Home visits give us the opportunity to assess a senior in a true life setting and get a “whole person” view of their health and wellbeing. We can address the challenges and appreciate the rewards of being at home. From the refrigerator check, to the blood pressure check, from discussions about the pictures on the mantle to moving to a more organized medication system….the opportunity to make a senior’s life at home safer and happier is a rewarding Practice.     I’m a big fan of home visits for patients who are more complicated or frail. I include them routinely as part of my family practice. I think one of the biggest obstacles to more consistent access to home visits is that we are not always well supported by home care services. In Ontario, there is no reliable way for home care nurses and others carers to communicate with a patient’s doctor. A PSW or other attendant might be the first to notice that a patient isn’t taking their medications appropriately, and yet it seems like there’s no real avenue for them to feedback their observations to me. I think we can learn a lot from palliative home care teams, which in my experience are often much more organized and inter-cooperative.
