New Territories: Apple Juice for Gastroenteritis in Children and Rheumatoid Arthritis Long Term Outcomes

This week, Amol and Janice, staff internist at Mount Sinai Hospital, discuss two studies:

A randomized trial asked whether diluted apple juice works just as well as oral rehydration therapy in treating minimal dehydration in children with gastroenteritis. Children 6 to 60 months presenting to the emergency department were randomized to half-strength apple juice or apple flavored maintenance solution. The study found that half-strength apple juice was not just non-inferior, but actually superior to oral rehydration therapy. Janice and Amol discuss limitations to the study’s methodology and generalizability.

Long term outcomes of patients with early rheumatoid arthritis were assessed in a 10-year trial in the Netherlands. The study found that patients with tight control over 10 years achieved good clinical control and functional outcomes, and there was no difference in outcomes or side effects among the four treatment strategy groups. Amol and Janice discuss the applicability of the study to clinical management today.
